My friends female GSD is about 3 years old and she weighs 55 pounds. She is pretty tall, about 26 or 27 inches at the shoulder. My friend has tried different foods...bil jac, evo.... she has brought her to the vet, tests have been run on her kidneys and such, because the vet is the one who first had a concern, she is spayed, she was spayed when she was younger, yet she constantly licks her privates...

Any ideas of what may be the problem??

She might have a yeast/candida thing going on. Around the vulva is a common place for irritation. What about her toes? Any redness between them? Or any other place on the body with skin folds?

Is she really only 55 pounds and 26-27" tall?? measure height at the shoulder to be sure. if she is only 55 lbs, she is extremely thin and that is death weight for that tall of a dog.
Does she eat regularly? and does it just go straight through her?  Did the vet check for bacterial infection or Whip worms?
Whip worms do not always show up on a slide, but  they are devastating to a dog. they can have a load of whips and pass few eggs to be picked up on a slide, sometimes no eggs on a slide at all. They can also develope a bacterial infection in the gut due to these worms.  One other thing is do you know if she ever had Parvo as a puppy/youngdog ? if so, she could have damage to the intestins from the virus and just cannot absorb the essential nutrients her body needs. If so she will need a very high quality dog food preferrably a puppy food, with less fiber in it to put on some weight. maybe let her eat small amounts whenever she wants instead of just a bowl or two a day until she puts some weight on.
Is she on Heartworm prevention? Or has she been tested negative for Heartworms? alot of things could be going on with her.
Keep us posted.
Bonnie.
