So I posted about my 1-year old getting her lepto vax and having what seemed like a bad reaction to it. Now after nearly $4k in vet bills and a week in the hospital, the vets have determined it is sialadenitis. I just about lost it when I saw how big her head had swelled since I saw her two days ago.

IMG_1384  She is now on phenobaritol and prednise and may have to take the phenobarb for the rest of her life.
Has anyone ever had a dog with this?

Wait, what? Why would they put her on Phenobarb if it's a bacterial infection? Is she swollen everywhere or just the face and throat? Sorry, I don't know what would prompt this after a lepto vacc. It's usually caused by like a trauma or a disease like rabies or distemper. I could be wrong, but I would get a second opinion. What all did they do to charge you $4k?

that poor baby! the pictures just showed up! I'm reading up on a study of it responding to Phenobarb, but Phenobarbitol usually is used to treat seizures, so I'm a little confused. And this thing is like fairly rare, did your vet send out samples to diagnose?

vet agrees it is a rare case. The $4k is the total so far for a vet visits every day since last tuesday, blood work, xrays, ultrasound, meds, and three days in hospitalization. Apparently(from what they say) there can be a salivary gland inflammation without known triggers and it usually responds (again for reasons unknown) to the phenobarb. The prednisone is to help keep the swelling down until the phenobarb kicks in. It is given at a much lower dose that for epileptic dogs so after she gets used to it, there should not be the same out-of-it quality that many epileptic dogs show. While the symptoms showed up almost immediately after the lepto vax, they are convinced it is unrelated. Her poor little muzzle is now 15 inches around.

I'm with Siantha on the second opinion. I know you guys have spent a lot already, but it could be something little, or something that's not going to get fixed with steroids and phenobarb. I've worked at 2 different clinics, with 6 vets at each clinic, and each one has a different say on every case, and each one always thinks they're right. Couldn't hurt to make sure.

I decided to Google this diagnosis and I came up with an answer that supposedly came from a vet.

Quote:

I have seen a handful of sialadenitis (also sialadenosis) cases over the 11 years I have been practicing. All were definitively diagnosed by biopsy. All responded rapidly to phenobarbital therapy.

I did have one patient who was also treated with prednisone as an underlying vaccine reaction was suspected.

Am I correct that prednisone is usually prescribed for autoimmune issues?  My son had a reaction to a vaccine. They tried to tell me it wasn't, but he was put on prednisone as his immune system started to kill his platelets within hours of having a vaccination. I would suspect a reaction to the vaccine. How the heck could it not be a vaccine reaction if it happened right after the vaccination?
I have heard that the lept vaccine has the highest adverse reaction rate and the vaccine is not that effective. I don't give it. Mind you, we dohn't tend to have the hot wet climate it favours.
Has ahyone heard anything different?

Hope your dog recovers quickly. I wouldn't be giving another Lepto vaccination ever.
