I agree with marjorie, having been there done this twice with two different gsd's.. Find yourself some DESITIN as in baby diaper rash creme..Put it on a q tip IN and around the fistula twice daily (as well as keeping the area clean/shaving up the area)..

Both of my dogs had PF's resulting from anal gland issues..The first one was years ago, had radical surgery removed the glands, problem disappeared. 2nd one was the result of a blown gland, I went the cyclo route and desitin, again cleared up..

If the dog has PF's as the result of anal gland issues, removing the glands may be a good thing, tho many vets don't want to do that, seek out a board certified surgeon who is savvy in this type of procedure.  

Also would like to mention, since you've been using cyclo/keto with no positive results, it may be the dosage..If you don't give enough, it's not going to work, if you give to much it's not going to work..Did your vet do any trough testing for dosage?  Anyhow, good luck, it's a nasty disease, but I think you'll find alot of help on the site I linked.
