Pano   comes in when a dog grows too fast , its all about  the dog , it has nothing to do with vac and or size of the dogs bones, my dog only is average size and went through a very painful time with pano  till 2 years and yes every time it got worst , there was times he  was in so much pain he would not put any pressure on his leg , poor guy and even got to the point of getting fevers, i swear by Ester C and i will always use this if i ever get another with pano, it has to be Ester C not any other vit c tablets,  it will work, but will take a good week to get enough into his system. this is what my experience is and i found out about Ester c from a friend, i  did try  other things  that the vet said and never did work. well as i tell people what do you have to lose to give Ester C a try for a few weeks , sure is alot cheaper than all the stuff your vet has tryed.  pano is  growing pains, so like others have said you need to ride it out.
