The pup is more likely to have health problems due to being a true runt, but that's not a guarantee...it's also possible that she'll be a healthy dog too. No way to tell what else was affected, if anything at all. Do make sure she gets supplemental feeding so she has the chance to catch up, without it she'll be much more likely to have problems. (would recommend supplementing the entire litter since it's so big)
edited to add:
the larger pups of the litter may not need additional supplement feeding right now, but with a litter this big you should definitely start them on soft puppy mush early, I have seen pups sampling food on their own as early as 3 weeks. When they reach 3 weeks start offering them formula/mash, if they're hungry they will eat it. Mom will need all the help she can get feeding those pups, lol.

Well, the little girl with the white legs has just passed away. I have been tube feeding her and a few of the other smaller pups (none as small as her), but she still wasn't growing and was not really even trying to nurse all that much. I am sad that we will not see her grow up, but glad that she is not suffering.

Well, I must say I was really intrigued by this post when it first came out, and even more so now... AS THIS SAME THING JUST HAPPENED TO ME! I can only hope our results are not the same (p.s. op, sorry for your loss).
A litter of 11 yesterday West German/East German cross. Smallest pup in the litter, same situation (i.e., absolutely no way another dog was involved). Pup is nursing well. I am just thankful I had this to refer to ....

Wow what were the odds of that happening? How about some pictures? Good thing Lil White Legs is nursing well with the others. Eleven is a lot of puppies!

I whelped a litter of pups for a Shepherd importer/breeder, here at the clinic.. and the first pup out was a smaller, almost all black, pup with 4 white legs, white chest and a bob tail..!!
I thought.. ""holy crap"".. an Australian Shepherd got ahold of this poor ladies bitch !! (Even though I know we did the AI and the female was never, ever exposed to another male)
The pup died shortly.. like within 20 seconds.. The rest of the litter were all normal size and of the correct color.. Strange.. something genetically was predisposed in it to not survive.. which was fine for all!! It was just one of those moments !!!
