I don't know the answer to that question scientifically, but I suspect the last puppies up in the uterine horms don't get as much nourishment in utero than the rest, which is probably why the biggest ones are usually born first and the dead ones born last. Just a guess after 30+ litters.

I think timing is part of it personally.
