I'm certainly no expert in this, or on the Serbian alphabet, but first off, the characters sure look more like Cyrillic [Russian] text than Serbian/Slavic...

Secondly, web search engines are your friend, especially if you're considering buying an animal from another country, sight unseen and where you don't speak the language or know the country's standard practices & documentation. A search using the term "serbian hip dysplasia certification dogs", provided a link to this sample of a hip & elbow certification issued by the Kennel Club Republic of Serbia, and it's not what you've posted here.

And interestingly enough, I see you've asked a similar question here two years ago, regarding an FCI registration, also for a Serbian dog, that showed the elbows had been rated normal, but there was no rating listed for the hips.

I agree with the person who said the document you posted today looks to more likely be a health certificate or 'passport' that documents the dog's vaccination status.

thanks Hexe, I appreciate your help. I agree, of the dogs I've imported before, there was a clear A stamp on their pedigrees, not this form!

This document is for sure not Hip or Elbow certificate for dogs in Serbia.
It is a simple pet "passport" containing basic information about the dog. Pages 2 and 3 of this document shown on your pictures are just sides which show that some veterinarian has put some type of identification on this dog (tattoo or microchip). This is a simple document created at a private vet officet. That is the most easiest paper/document to get there and it costs around $140 ($1 = 85 dinars) and includes all examination, vaccinations and paperwork.
Here you will find more info on what this document is about: http://www.google.com/translate?hl=en&ie=UTF8&sl=auto&tl=en&u=http%3A%2F%2Fwww.zivotinje.rs%2FvestiStrana.php%3Fid%3D5893%26name%3Dkako-i-za-koliko-do-pasosa-za-ljubimca-cenovnik%26tip%3Dvesti

Hexe posted a good example of real him&elbow certificate issued by the Serbian Kennel Club.

Following is also an example of official pedigree papers issued by the Serbian Kennel Club  - example is for "Middle-Asian Shepherd" papers but the same is applicable for GSD.

Also looking closer at the document you posted this pet passport was issued by the vet office called "Pan Vet" in the City of Subotica (Northern Serbia, close to border with Hungary).

It seems to be a legit veterinary office: http://www.google.com/translate?hl=en&ie=UTF8&sl=auto&tl=en&u=http%3A%2F%2Fwww.panvet.info%2Findex.html
