I went back to reward for outing when told. Got great results compared to punishing for not outing.
I've seen dogs that did not fully understand what was expected when told out, especially if that leash was not attached. It's amazing how a person handling can telegraph up the line to the dog, let go of the line, or take it off, and the command is not understood.

first step is to have him perfect in obedience , to out on the ball and dumbell perfect , without hesitation or fight
second in bitework you can work a dog like in the beginning of this clip , exchanging the sleeve or pillow time after time to program the dog

third , get a good helper , that can give right signals to the dog and correct him when you are on distance
when everything else failed you can use Ecollar , but dont use it when the sleeve is on the helper , let your dog win the sleeve , walk away from the helper , , give command out , and if ignored , shortly after command give a short pulse
until he does that perfectly , dont go back on outing on the helper
