Dawgma, that's really not enough information to even speculate.  That could be anything from an insect bite, an injury to something a whole lot more serious.

I'd vet him.

The correct answer is:  take your dog to the vet!

My goodness, can you imagine how uncomfortable, if not downright painful, inflamed testicles are for your poor dog?  Get him professional medical help, THEN come back here and we all can discuss causes and cures.  HELP him, he's totally dependent on you to relieve his pain.  He's counting on you!

I have seen this problem with a German Shepherd before.  It sounds like something has set up an infection in his testicles from a cut or from something irritating his testicles and caused them to get infected.  You are going to definitely have to take this male to a VET right away to get antibiotics for the infection, because if you don't you may end up with a sterile male if you are planning on using him for breeding.  Have you ever wondered why some dogs are sterile.  Get this taken care of right away.
