The best airline would still be Philippine Airlines (PAL) since it flies direct from LA to Manila. Call them and ask for the requirements. I think they have an office in LA.

I sent one 4 to 5 years ago, you are lucky you looking for LA, there is only 1 usda vet in the area. You can call them and and get the proper papers to ship
