I would probably treat him with ivermectin..  even if you don't find mites.. it looks very suspicious..  hopefully your vet will make a positive find with a scraping..   good luck with it..    it does take time to totally clear up..

Demodex won't always show up on a skin scrape. If they don't get deep enough to get a mite in the sample or hit a place with mites, you will get a negative result.  Demodex itself isn't very itchy. Sarcopic will drive you insane itching.
How's his feet look? Down between the pads and toes and around the nails? Demodex loves feet.
I would be looking at some other possibilities. Bacterial, fungal or yeast infection (or any combo) going on as well.

My son's vet told him that in a young dog the prognosis is excellent. My son's dog did not show improvement immediately, so don't get discouraged. He also had to be on antibiotics for a while.  Hopefully you will get a concrete answer soon.

Did you tell the breeder about this? If so, what did the breeder say? According to my son, his vet told him that this is genetic and the parents should no longer be bred. I would ask your vet about this just to be sure this is accurate information before you say anything.

His feet and toes haven't been affected at all Just around his legs. He did have a fungal skin infection around the time we took him to the vet for his first skin scraping and it cleared out, none of this seems to be of the same degree as that but is is definately something to think about.

Again, I'm just kind of guessing until I can really figure out what it is. I'm just tired of seeing my poor boy so miserable.

Folks here are right. Get him back to the vet, do some more scrapes. Have the vet do both the legs and places on his back. The hair loss around the eyes sounds like classic local demodex.  Ivermectin will clear it up but it will take time. And test for infection. Beating demodex requires beating any secondary skin infections as well. Antibiotics, antifungals, etc. If your vet can't give you any answers, find another vet.
Regardless, his immune system is compromised by something. The legs though look like there is something else going on. If skin scrapings come back negative again, I'd still have this guy on some meds.
