I have a copy of such papers and the German hip/elbow/dna stamps are on the cover page of the pedigree, not the inside pages where the pedigree is shown.

No, it clearly isn't a hips n elbows grading !

All UK vets issue record cards like this, for proof of vaccinations.
Have never heard of any separate charge for it, here.
It's a document you need, to prove immunisation, whenever you
want to board a dog (or cat) in kennels.
Usually they include a few more basic details:  name and address
of dog and owner, when they first attended that vets practice, etc,
& you can fill in other stuff, like colour, tattoo or microchip number
yourself.  When you change vets, you usually get a new card from the
new surgery.

Even though the translated parts call it a 'passport', unless there are more
than the two sides shown in the OP,  it probably isn't actually a "pet passport"

for intercontinental travel.   In our case  we have a different document for travel,
EU and otherwise, showing Rabies injection and worming etc, to comply with
legislation about those things, as well as ID #.   [The 'powers that be' don't seem
so bothered about the routine things, like Parvo, Kennel Cough and Distemper. LOL].  
I don't know how much they require for travel from Serbia, but would guess that EU rules,
applying across the board, would mean more spaces on this card, if that IS its purpose,  
even if they have not needed to be filled in for export to the States ?  (Can't imagine they
go to the trouble of making two separate cards, one for Europe and one for anywhere else
on the planet.)
