Just another thing here,

All 3 things are done on the one day,

They start with the x-ray,  please tell me who reads the x-ray the vet who takes them of course. How many vets are out there that can really tell if a YOUNG puppy of 12 - 20 weeks does have or has the potential to develop HD ?
We x-ray our dogs at a later age depending on the country where we live so that is somewhere between 12 - 24 months, yes the vet will look at them a give an opinion but what is that worth ? How often is owner told the hips don't look good ? 
I was once told this by a vet who had to take more than 10 x-rays to get one worthy to send for evaluation, the vet said He didn't think they would pass the result was a total score of 6 here in Australia that is great.
What I am saying is that most vets are not GOOD at grading hip x-rays, that is why we have to send then to a specialist for evaluation.
That being said the vet that takes these x-rays and looks at them and decides if the puppy requires surgery without waking the puppy and goes right ahead and does the surgery. My bet is that none of these puppies ever leave the veterinary hospital without having this surgery.  Why even bother to x-ray ??

And at what cost to the owner I wonder?  Working at a vet for all these years, I can't say that we see an "alarming" number of hip dysplasia.  See more torn cruciates in Labs and Goldens more than anything!   It almost seems that we see less dysplasia, but now there's a "new" problem with the cruciates.  Did recently see a very sweet Golden from a pet shop, that was an orthopedic nightmare!   Bad hips and elbows, and a bad shoulder.....And a GSD with bad hips....We also don't xray every large breed dog's hips either, so maybe there is more, we just don't know about it.....We do OFA xrays and xray upon request or if the pet is having problems....

I am also a vet nurse, though I don't work that often anymore but you are right we don't see that many dogs with HD, far more cruciates ligaments these days.

Tear one cruciate and are almost guaranteed to tear the other within a year.  We see so many happen in young dogs too.   Sometimes they tear both at the same time too!  You can almost pick them out of the waiting room too, as they are walking in!   Sad but true!   Then if it's a Rottie over 7 you think bone tumor if they are limping...We see a lot of rescue Greyhounds too and for those too, bone tumor is high on the list as well.
