Sorry for the lag time, medical grade pure lanolin should work as well, if you can't find bag balm! Should be able to get that at any drug store, or any preparation made for udders/teats found at feed stores would suffice, apply sparingly. Shouldn't apply anything you're not comfortable with the puppies injesting. The bag/udder/teat treatments are made to be used on lactating animals, & thus safe for offspring. jh

I would  have her lay on her back , rub her belly for a while  to make her comfortable incase she doesn't like it. 
I would use some Hydrogen Peroxide, put in a small spray bottle, so you can mist it on the red inflamed area,   LET IT DRY!  The Great thing about Hyd.Perox.  is that if there is any bacteria or infection, it will be obvious by the white bubbles.     Once the  Hyd. Perox.  is completely dry  ,  Put a very generous layer of  " Desitine "  .     It can be found in the  Human baby section , just about anywhere.   

Hydrogen Peroxide , followed by Desitine, is also fantastic for Hotspots.
Desitine is healing and Protective at the same time.

Desitin is used for diaper rash, & is good if you can assure that dogs do not lick, or nursing puppies do not nurse it in with mothers milk. Desitin contains cod liver oil, no problem...but it also contains zinc oxide, not so good to ingest, especially for little pups. Great for moist areas that can be protected from licking. jackie harris

From what I interpret, the problem is up in the moist area between the teats where the bags of milk are so heavy that they are most likely sweating and there isn't any air getting to it.    As, I have dealt with a well indowed mama before. LOL !! 
That female had enough milk to feed a cow !  

I wouldn't put it on the nipples, where the puppies would be nursing,  The puppies are not very likely to want to try to nurse on it though. 

I would caution though, that if you do anything medicinally, that you want to think about things that will " REDUCE MOISTURE "  in that area , verses creams that will hold moisture.   If nothing else and you don't feel comfortable with the Destine, then just at least pull her out several times a day and clean the area , mist a little Hyd. Perox.  to keep down bacteria and allow to dry.  Maybe add a touch of baby powder very thinly.

It is brilliant for skin irritations, hot spots and the like and doesn't do any harm to the pups.

I've found that using Aloe Vera based spray has been effective at keeping the skin clean and helps any sores to heal.  I'm always wary that things will cause upsets to the pups.  The aloe will also cool any heat spots.  

You need to keep the skin supple as it is expanding and contracting each time the bitch gets a rest and then feeds the pups. Try gently wiping the crease between the two rows of nipples each time you bring her away from the pups - removing any moisture whether it be from the puppies/milk or sweat.

Calamine lotion, works for most hot spots and wet rashes on my dogs and wont make pups sick can be put on as much as you want, just pat it on and dry ,blow or put her out to dry. Just a few minutes done. It most likely is from moisture with teets large and drooping, but pups will be drawn to the smell and lick thinking it is a teet full of milk as it is wet. Cant hurt to try and is cheap at all groceries and pharmacy's . I use it on any thing wet, hot and itchy,so I have big bottles. She will stop nursing if you cant stop the pain. best to all
