@Kies, I imported a puppy in April of this year (so, last month) before he was four months old from a country not rabies free according to the CDC. I contacted the CDC, let them know my circumstances, and they said that I was approved to quarantine at my home, but they needed to know when the puppy was coming into the states.
I let them know, but by the time they got the message and replied, the quarantine time limit had already been exceeded and his rabies vaccination had already taken effect, so they said no further action was required. (So really, he was never formally quarantined, but I followed all the quarantine relations which states he can't be around any animals or any people unless it's for care.)
He had to go through customs. You can hire an agent to do all the paperwork for you in the city the puppy will arrive in. After the agent clears the puppy through customs, the puppy will be able to be shipped to your city or closest airport. (I didn't know this, so I drove forever to pick him up myself and took four hours trying to clear him myself.)
I also contacted APHIS, and I believe they said that they didn't need to do anything and for me to contact the CDC, which I had already done.

The puppy still needs to be cleared by customs. Also, if you get more than one or two puppies a year, the customs officer said they would consider you a business and you'd have to go through some extra loops, but he didn't tell me exactly what they were. It's just safest to wait until the puppy is four months old and have his vaccinations done.
