Forget the shampoo. If it masks Ivermectin will clean a dog out, get's rid of a lot of stuff but I wouldn't consider it treatment. What you really need is confidence that your dog is healthy after the vet probably put enough doubt in the situation to really make you wonder. Do this by just doing what naturally fixes it. Give him the nutrition he needs to combat it himself. He's young, going through puberty and he has acne. His immune system is depressed and chances are he just needs a good diet. When he heals himself with a diet change, you can send your breeder an apology and find a new vet.

Change his food. Especially if what you have been feeding him is the same as he was being fed 6 months ago. Kibble is basically crap. Especially crap if you're trying to prop uf a depressed immune system. Lots of good raw recipies on line. When I feed my dog a chicken thigh, I know what's in it. The two main ingredients are just what the label says, chicken and thigh. Simple. No list of chemicals, preservatives, and by products from the rendering process shipped in from China.

If he were mine, before I'd rant to or about the breeder, I'd look at everything in my environment. When was he last dewormed? When was he last vaccinated? Stuff like that seems like no big deal, but it can often be the catalyst for something like localized demodex which is what you're describing. This dog isn't bald- he has a few spots. Like Dog1 said, he's a teenager with acne. While I take demodex very seriously and would NEVER breed a dog who ended up w/generalized demodex or who often had pups end up with even localized, I wouldn't throw the baby out w/the bathwater if ONE puppy had a mild case of localized demodex.

Hexe's post pretty much said it all.
