Agree with Lynn & Hexe. It's great his energy levels are up, and so that indicates that he feels a little better in himself, but he is still not managing to use all his food, and I think the vet needs to know this sooner rather than later.
Did you weigh him when we were first discussing this, and if so what does he weigh now? This will give you a very good indication of what progress if any is being made in weight gain, which is extremely important for your young boy.

Things have become bad to worse. Now he is not able to use his food at all. Stools are just loose with blood in them. I am frustrated. However, the energy levels are good. I did deworming with drontal, gave a course of metronidazole for 7 days (600 mg t.i.d), Gave ivermectin tablet as single dose. Now he is on Lixen Palatabs since 1 week because of skin infection. Also I give him Creon 25 five minutes before meals two times a day.
But my younger pup who is 4 1/2 months old eats the same food, drinks the same water and plays in the same enviroment is in superb condition.
Now I have seperated both of them as I cannot afford my younger pup with similar infection.
Please Help ASAP or else I may loose him..

There is no question about it: he needs to be seen by the good vet in the bigger city as quickly as humanly possible.

Here is my last request from you. Could you please post a picture of your creon bottle and tell me the amounts of each enzyme. I have been assuming that your creon would be the same as mine, however, because you are in a different country than me, perhaps they are different. Are they pork based enzymes? and what is the strength.
When my girl was first diagnosed with EPI my vet had me buy Royal Canin Gastro Intestinal High Energy Kibble and canned and she ordered pork based Pancreatin enzymes.
I was to soak the kibble and add the canned food, then add the enzymes and it was to sit for 20 minutes until it became soupy. ( that is how you know the enzymes are working ). the enzymes are predigesting the food in the bowl and then the dog eats the predigested food. I now make my girls food up at night for the next day and it is so much easier and the enzymes are less caustic on her mouth that way as they have been sitting so long and she is just fine eating it cold.
Maya was given a Vitamin B-12 shot at that time. She ate 4 times a day 2 cups of soaked kibble with about 1/4 can of food each feeding and 2 teaspoons of enzymes.
By day three she was normal for the first time in months.
Now, until you have the TLI test we will never know for absolute certain if Mervin has EPI or just needs some help with digestion.
You really need to get Mervin back to that vet in the city. Bloodwork needs to be done to check the TLI number and the Folate ( B-12 ) number. Your dog cannot afford to lose any more weight at this point. If in fact it proves to be EPI then I would suggest you do exactly like I did with my girl. Start with the exact Royal Canin food I said above, the canned and kibble, you can later change to another grain free food, but the weight has to go back on your dog now.
You should call the vet and tell him you want him to order in the Pancreatin enzymes ( pork based enzymes not plant based enzymes ) and you want him to have on hand that food when you come in just in case Mervin has EPI and you need to not come back into the city for it, as it will be there for you.
There could be more going on with Mervin, I don't know, but from everything you say on here I have to go with what I think is wrong with you pup.
I apologize if I am wrong because I am not a vet but you are crying out for help and this is the help I can give.

Agree with Hexe and Lynn. Get him back to the vet immediately.
The Creon 25, the pancreatic enzymes must be given with the food, not separately, if it is EPI. How long have you been giving him the Creon? How this is given is extremely important, as Lynn says, I don't know but logic tells me that it could be quite damaging to the stomach lining and intestines to give this without food.
In any case, firstly, you need to see the vet without delay.

To the OP: If your dog is having problems, take him to your vet and do exactly what he/she tells you to do. In general, most veterinarians are pretty good if they have maintained a healthy practice for a number of years. You just need to trust in that. No other advice on this forum will help you save this animal because it’s impossible for anyone to help you without seeing this animal. It’s that simple. Good luck and by all means keep us posted as to what the vet suggests.
