  Keith: I hope you never get crushed or lose your foot or leg or arm .YOU have no idea what others do nor do you walk in their shoes.

  HEITH has no shoes..HE cannot wear shoes.. THEY Cost 1,999.00 for a pair of them  52 Doctors have refused to take  him as their patient as he is A WORKMANS COMP patient.  HE has that coverage but it is like bubble gum stuck on the underside of a chair...IF a dr does not want to see  you and is approved as a dr to use WORKMAN COMP system YOU cannot see that DOCTOR>

 THE insurance company is CNA>..THEY deny all the treatments that Aaron Caladoney and DR Daniel PHelps tried to keep up with and the drs finally said....WE cannot treat HEITH any longer..THE arguments with WC and CNA are taking up time in our office and I wll not subject my secretaries to any more of WC and CNA nasty refusals and arguments..I have the last letter stateing that to cna.

Immediately Dr Daniel Phelps issued same letter and stated it is now time to remove the whole foot as to even assure Heiths life can go on in half a normal capacity.. IT is not known if Heith will ever be able to resume his employment to INdustrial Electrician..

ALL His doctors have stated , he is a hard one to tie down to even heal his wounds..HE does all he can till the pain is so bad he has to take a pain pill.

 YES< It costs me some to keep him in electricity..but the tree he cut down today....all the contractors in Smith CO are charging 1000.00 a tree of that size to cut...Texas has tons of dead trees to cut down as the drought killed them on all our lands..

 HEith has told Dars over and over..I do not want the DAMN GOVERNMNET disability..HIS doctors insisted he file and Dr. BLaue ins Tyler spent days having his nurse call drs in 3 counties to treat him..NO LUCK
 I have that letter MR KEITH stating the denials to that nurse and the Dr who tried.. MANY have tried to find him a  Doctor but you cannot make a dr treat you or cut your foot off. It has to be an orthopedic surgeon andi because he is an awarded WC medical recipient you have to wait till one is found...WE have waited thru 6 years now and going on 7..IF you find a dr who will cut it off you have to have the money up front..DO YOU have 20,500.00 for the BILL>>>???

 I do not..I nor any member of my family has no need to pay for something that a PLANt IN TEXAS CITY TEXAS DID TO HIM..THE DOOR HINGE MADE BY THAT COMPANY BROKE.AND THE DOOR WAS MADE BY THAT COMPANY...SO THEREFORE...YOU CANNOT SUE THE COMPANY AND HE GOT NOTHING...HE DREW WORKMANS COMP PAY FOR 2 YRS.

  BY the way Keith...MY son is an adopted son...  your discription of my son LAZY HELL=SPAWN?????????   I BIRTHED MY DAUGHTER AND MY HUSBAND IS DEAD. MY SON WAS ADOPTED WHEN HE WAS A BABY. 
 
Also for your information...Heith was 18 yrs old when the lead plate door fell 30 ft from above as he swept out the Sand room., crushed and under the door for over 30 minutes till the closing gateman realized he had not checked out. HE lost big toe , 2nd toe and 3rd toe, foot crushed his leg half distorted and trapped under what the Oscha called  A GUILOTINE , rushed to Gal CO hospital and then to St. MARY's In Galveston and later transferred to St Josephs under Dr Donald Baxter, top orthopedic in Texas who was his surgeon till 2000 when he referred him to Tyler Tex. He still has night mares to this day of this event.

.Mrs. McWilliams has phoned, and written so many drs offices begging them to see HEITH it is unbelievable..
She will not give you facts but she will tell you Heith has been thru HELL and back with his injuries and required help that his insurance com has denied..she has proof and I have it here..SHe sent it to me last week of partial phone call records of  her proof he was denied by his Comp carrier which entitles HEITH to sue CNA>>>HE SAID    SCrew CNA>..I want to walk my dog in no pain  and I want to walk and learn to domy work again if my body will let me..

Heart attacks , KEITh IN CASE YOU DO NOT KNOW... HALT THE BODY  from any kind of repair to a surgery so we wait again till now..

I have tons of proof of that...I have former employers of Heiths that have letters to prove they watched him work in pain for hours and all his former companies including Hal Baker, were aware he had to get off his foot many times during a day..THEY allowed it.. HE wore special shoes paid for by CNA at that time..BUT now no shoes can touch his foot...it is a swollen mass of   dr. BAXTeRS term:  HAMBURBER MEAT>.

 NINJA has the letters from Dr Coladney and Sam Hawkins an employer he worked for after he could not go back to HAL BAKER>  he worked for an individual , a christian man who owned Hawkins Electrica and broadcasting company who keeps 4 stations on the air in TYLER> MY son did all the climbing the TOWER in RED MOUND , at 1,500.00 a climb because no one else was capable..even with TWO FEET...HE ONLY HAS 1 AND A HALF...HE WORKD TILL THE PAIN GOT SO BAD HE  was on his knees in pain and cannot walk..

so he is sitting on his ASSS ....I posted a pic of him in Kentucky , one foot missing as is now, picking up poop while he was working on that foot for Reinicke Athens... YOU live close to Sommerset KY...so maybe he passed you on the way thru Indiana...lol

THE LETTER ABOVE WAS FROM THE PHYSCOLOGIST WHO HAS TO APPROVE WHEN A FOOT IS REMOVED..it SAYS...HEITH  is in sane mind and has undergone my treatment for removal of his left foot crushed and half mutilated by amputation of members and a partial amputation has not let him do his normal activities.

 Heith has begged for 19 years for removal of his foot to no avail because of State Workman Comp and he is now of sane mind and I determine he is able to make that decision

He should live a better life by this surgery and I suggest it be done as soon as possible 

 that letter was to Dr Gary Rubin,  dated  March 2009, who only cut the foot in HALF right in the middle of the Crush..on the date listed above. which was done in May of 2009.

 Dr Catherine Walsh ,

WHen the time came after 1yr of healing to cut it off the rest of the way, DR. GARY RUBIN could not be found by ETMC in TYLER or CONROE>.

SO  we wait for a dr to be found to finish it...SO KEITH>

I hope  you do not get injured at the company you work for..but you live in a state that probably gives you a million dollars if you lose your foot and are crushed under a lead PLATE DOOR>>

I am waiting to hear back from Dr Cox’s office about whether they can accept him….Dr Cox can get him within a couple of days of them agreeing to see him. As soon as I hear something I will let you and Heith know…I know he is in a lot of pain and very frustrated and I am doing the best I can for him and pushing people as much as I can but I will keep in touch, let Heith know since they move me around a lot the easiest way to get in touch with me.

Hi Ms. Johnson,

 

I wanted to let you know that I have talked to Brenda twice at Ms. Tevebaugh’s office and Tevebaugh has been out on vacation.  She is back now and has not been able to sign the release of information request for the records from us yet.  Evidently, she has quite a stack of work on her desk and still trying to get through it all. 

 

Brenda assured me that the request is on her desk and that as soon as Ms Tevebaugh has a chance to sign it, that they will fax it to us.  I will be in Colorado next week, so I have alerted our Medical Records manager, Kristine to be looking for the fax so that she can expedite the delivery of these records to Tevebaugh’s office.

 

I just wanted to let you know where we are with this issue.  As soon as I return, I will follow up with Kristine to be sure that the records have been requested and sent, and I will let you know at that time as well.

Hi Heith,

 

I tried to reach you today by phone, and left a message but thought I would go ahead and send you an email with some information.

I have met you at The Gulf Coast Center and have talked also with your mom about helping you get a prosthesis for your foot. 

 

I talked to Jim Gilliam at DARS  (formerly TRC) who helps people with many different things, and he told me that you could be eligible for a prosthesis.  He recommended that you visit their Texas City office which is located at 2000 Texas Avenue, Suite 950 in Texas City.  When you walk inside the building, you will go to the left side of the building and go all the way to the back to get to their office and the sign will say DARS.  You will meet with a Counselor, Larry Gladden who will help you complete a form to request the prosthesis.  Mr. Gladden will assist you with all of this.  

 

Mr. Gilliam said that they will need to know that your medications are helping you, and that you are in compliance with your medication appointments.  I assured them that you were very compliant.   They prefer to provide the assistance after the amputation, but I’m sure that communication can be provided between DARS and your physician to make this happen.

 

The phone number for the DARS Texas city office is 409-949-3900, but you don’t have to make an appointment.  It would be good if you could go there tomorrow, Friday, and meet with Larry Gladden.

 

Please call me if you have any questions or concerns.  I will be glad to help you with whatever paperwork you may need from Dr Walsh or our Center.

Hi Ms

Oh My God, I hated to catch your voice messages late yesterday evening!!!  We were in a meeting till 5:30pm, and I picked up my messages remotely on my way home last night from work!  I just wanted to Cry when I heard your messages!  Poor Heith….Bless his heart!  He’s waited all these years to get this done and it finally was happening and this happens!!  I just can’t believe it!!!  I looked for your email, but did not receive it. 

I tried to call but your line was busy so I thought I should email to let you know that I got the news!

 

Maybe the Dr just needs to take a further exam of it for his own “liability” sake, and plans to complete the surgery the way Heith has asked him to do.  If there’s one thing I’ve learned over these years of working around physicians….it’s very much about the “liability” of their actions that orchestrate their actions…or not.  He has to be convinced that there won’t be any repercussion from his surgery before he’s going to do it.  Surely he can be convinced that this is absolutely what Heith needs and wants to happen!!

 

I’m so sorry for you having to deal with this too!  I understand  that this has been such a long and unbelievable journey that you have been on with Heith and it finally seemed to be coming to its end….and now this.  I can’t tell you how much I wished that hadn’t happened for both of your sakes!  I know this must be extremely difficult for Heith to have to face….and I’m so sorry about that and it makes me so angry as I know it does you! 

 

I will definitely make sure that Dr Walsh gets this message and is aware to be on “stand-by” to see Heith if he needs her next week!!  Again, I am so sorry that he’s facing yet another “setback”, but I know that we will do whatever we can to see him through this and to help him not give up!!

Keith:  Another set back  THIS ordeal has gone on for 6 plus years to find a dr to do it..again..the Dr refused to go forth..HE found out HEITH is a workmans comp awarded patient..

HE may not do the surgery..because of liability of being sued by a carrier of WC>..there are rules when you get hurt and are under WC.  YOU have no clue and you do not know any facts about what happens when and where..THE doctor can do it...take pay from another Place and refuse to let WC know...but the minute the Prosthesis company bills  WC ..the want the hospital records to prove his foot is gone...so  the DR> withdraws from doing the surgery...SO DO  YOU UNDERSTAND ANYTHING>>>
