Another thought as I can't see your dog's butt - pics are always good - and you said it was all red, could this be a hot spot or other skin infection? That might explain why your vet gave meds and didn't check anal glands.

In addition to everything already mentioned, allergies and yeast infections too, can cause an itchy anal region. If the bum-licking is in conjuction with any other signs like foot-chewing or ear-scratching, that might be an avenue to consider.

Eskimo, I am concerned that you say your dog was given meds but
they didn't work. What were they ? And how long did you give them ?
(most drugs can take a while to 'kick in').
If memory serves, the first episode of a longstanding case of fistulas in one
of 'my' dogs was treated with cyclosporin (in the human variety, this was a while
back now, before the veterinary drug was cleared for use) for over three
weeks before the holes closed up, and he had to go on taking the capsules
for a further 2 (?) weeks even then. In later recurrences, the condition became
harder to treat. So AF /PF can be a very unpleasant affliction. OTOH, I've
heard quite a lot of stories from people whose dogs have just had it the once,
got on top of it, and never looked back. So there is hope. If AF is what is wrong.
Without seeing the dog or more detailed description from you it is hard to guess.
