Let's start over. Find another vet office to take him to for practice. Start with treats in pocket and take him in and sit. Give treat. Wait a few minutes; TALK TO N O ONE; NO ONE PETS HIM. Get him to focus on you and only you. The WATCH ME COMMAND. If he does not do that for you; leave. Take him home and teach him to focus on you and ONLY YOU.
Now that he focuses on command go to a local store; sit out front with him focusing on YOU and ONLY YOU. NO one pets him. He is working..... Do not distract him. Don't ask him to act like an adult as he is not an adult. You have to go back to the beginning and recreate and re-focus his mind and attitude. It won't take long if you are diligent about this. Any time he misbehaves you start over. Eventually he will get it that you want him to focus on you only. Put him in new surroundings every time you go out. Stay out of the vet's office for now if you can.
My boy actually chased the vet out of his office once. What a naughty boy. I was surprised and not focused enough myself to give him the proper correction at the time and created a monster for myself and him from then on which was 6-7 years of hell. All escalated into him having to wear a muzzle at the vet's and anywhere else he did not want to go. Not a good scenario. Obviously something went on and you will probably never know. OH well. Move on.
I hope this helps you and your boy. The watch me command will stay with him for the rest of his life; anytime he feels insecure get him to focus on you with his head turned toward you focusing only on you. Teach him that is his safety net. Best of luck Nan
