obviously something happened during that part of the routine that triggers this behaviour

I would do the next
first make your choice of reward , that she likes best , ball , food , biteroll

next , make her sit in front of you  , and offer the dumbell , iff she holds the dumbell make her stay  and YOU change your position to where the dog is in the heeling position
make her out the dumbell and praise/reward
take half a step forward and make her heel , if she does so  praise/reward  her like she won the WUSV

if this goes well go from half a step to a whole step , to from behind you  till all the way around
from in front of you
dont go to fast , 1 or two times good is not good enough for the next step 15 times or more is

This is a very good question....because it is not unusual at all.

I have the other problem with this exercise with my dogs.
But the people who are new my be interested in knowing that
sometimes, especially with high drive dogs, you my have to
make the dumbbell distasteful. The Germans alot of times will
use a old dumbbell with small nails hammered into it. So
it is not so likable( distasteful). Those little carpet nails work good. This
 is for a dog with high drive, but poor follow through. You are after focus.
 
I would have to see your dog do a couple routines to be
able to advise you.
