HI When I worked for a Vet we had an animal come to us with this sort of pattern it was inner ear infection which caused imbalance.
A sensitivity test of the debri in the ear was carried out and a course of antibiotics which had the most effect from the test were given.
This cleared the problem up.

As was stated, this dog doesn't seem to show any debris (YET). Much greater chance the dog has allergies (invisible, until they domino-effect to other injuries/infections) than a regular infection.

BTW - BENADRYL - don't worry about it. My dog has had 12 pills a day for some 6 mos. I was worried myself that since 1 of these little pills causes me to get very drowsy, 12 would knock her out (4 at 1 time). But it hasn't, even at the start; she is the same as she always was (hyper on greetings, over-excited about "Jolly Ball", and otherwise alert)!
Unfortunately Benadryl doesn't come any larger than 25 mg, so we have to give her 4 of those pills at once 3x/day. It would be so much nicer if 100 mg pills existed.
Hopefully, in any case, you could just try a smaller dosage; ask the vet for recommended starting dosage.
