You should be able to buy it at a feed store or on the net. No script needed. You should cut it with propylene glycol. A google search will give you all the information you need to do that. You can also use ivermectin horse paste. Again the dose for dogs is on the net. Be aware that none of these products are approved for dogs and some think that the paste is not a good idea since you only give a small portion of each tube and they ivermec may not be evenly distributed throughout the entire tube.

I get the liquid form online from Amazon.com and had a chemistry professor help me figure out the dosage.  I used to get the paste from a farm store but the liquid is even cheaper and far more accurate for me to dose.  I dose 8 times higher than a Heartgard tablet.

If you get sheep drench you don't have to dilute it, same with the Ivermectin powder.

Correct dosages for Ivomec, Ivermetcin powder and sheep drench:

Paste is too dangerous, too easy to overdose and horrible if your dog somehow gets the tube and chews it.

Mix any of the poweder or liquid in correct dose with Mac and cheese.
