I am curious to know how much a dog would be penalized for coming off the sleeve because it hit it too fast. If it was obviously committed to the bite, and just came in too fast, then attacked again as soon as it was able, would it cost the dog much in the way of points?

not dog should be penalized but rather those, who established rules and criteria of IPO to distinguish most perfect from their point of view "working" dogs.
last my dog back in 1994, was disqualified when we challenged third level of schutzhund.

unless the dog thought he heard an out command from the handler then i would not judge the dog as hard

nope, dog not lacking the drive or scared. if he was so, he would run away or do circles around handler.
after all, he did to this level of competition.
it more looks like dog was confused and lost.

that's why IPO dogs not really fits to anything other than sport.
dogs, able to do real work, wouldn't pass even to local/regional level.
