Five weeks is too young....the mother's antibodies are still active in the pup, the vaccine at five weeks is cancelled out by the collostrum antibodies from the mother. No earlier than eight weeks old for first 5 way.
Four week intervals two more times. Then at 16months old one more 5 way...that is all !
Rabies according to your state laws.
Vaccinating too young, too many times is causing autoimmune issues in dogs. Stop over vaccinating, and vaccinating before the pup' s immune system is developed enough to utilise the vaccine properly.
