Is it true, that OFA, has been known to usually fail dogs if they are over age 3?

No. I know someone who got excellent on an almost 4 year old and another person who got fair on an almost 6 year old.

I had a GSD female that was given OFA Good hips at 8 years.

As everyone else has already stated, OFA has no upper age limit for certification. If a 3 year old doesn't pass, it is because it has HD (or the xray quality was poor), not because of its age.

Probably. they put a dog with excellent hips as good and a dog with fair hips as good.

Do you mean your OFA rating certificate? Unless you specifically made your results private when filling out your paperwork for OFA, the results are online and can be retrieved by anyone with the dog's registered name or registration number I believe.
