karthago is one of the very few kennels that is producing quality dogs over several decades

My experience with GSD breeders of the world is limited, so I will not presume to write about "best in the world."  The track record of von der Staatsmacht is considerable and impressive.  Another North American kennel that I think deserves mention is "vom Kiebitzende" in Prince Edward Island, Canada.  It has an impressive track record and meets the criteria mentioned in the original post.

If i were looking for a sport or working line dog this is list i would be focused on as I believe they have proven themselves time and time again (Order is just how i wrote them down, not a ranking).

I never really think about certain Kennels being the best in the world (for German Shepherds)...I have never said it this way but have often thought it and have wanted to say that I don't really think about certain kennels being the best...The way I think about it is what dogs do they have and are breeding...I usually look at certain dogs that I'm interested in for one reason or another...Then begin looking at their progeny...No matter what kennel they're from or what kennel name they possess...

Now...saying all that...I do look at certain kennels such as:

Von Der Staatsmacht

Del Lupo Nero

Vom Fuchsgraben

Tiekerhook

...but basically I think of individual dogs NOT particular kennels...but it's also true that a lot of these kennels have some of the best dogs in the world...

 well proven tiekerhook have male and female from this line and never disappointed .bought dog from other well known kennels most of turned out to be shit .it not the kennel for me its the dogs that come thought .iv never seen a bad tiekerhook dog

SO THIS IS INTERESTING!!!! 

Aperently the Kennel's  VAN TIEKERHOOK, DEL LUPO NERO, AND VON DER TEUFELSKELHE have created a global co-operation breeding union in order to better the breed. 

3 very reputable kennels with excellent history and all working together.... I think that is a game changer kind of move. This is new news to me so dont attack me if you already knew this.

This is to newbies (like myself), best kennel, if you're buying, does not mean you automatically get a very good puppy or dog. Your dream puppy or dog may be at a very small and not so popular kennel. Just a side note.
