The reality of Sentinel is that it contains 5 times the dosage needed for heartworm control. This was a marketing ploy by Novartis to try and separate Sentinel from the heartworm only insecticides ( call them what they are ). You can reduce the Sentinel rate to 1/5 of the full dose in Sentinel and still get heartworm control. If you have more Sentinel tabs just wait 60 days from the last dose at full rate of Sentinel and then give 1/5 of a tablet recommended for your dogs weight. It is likely that your dog has enough Sentinel in his body fat and bloodstream to last 60 days since the rate he has been taking is 5X that needed for heartworm control. Macrocyclic lactones control heartworms 100% on a 45 day schedule and 99% on a 60 day schedule due to "reach back" control of juvenile stage heartworms that are microscopic in size. Adult dogs don't need monthly worming and the Sentinel dosage which includes Lufenuron was for worming and fleas not for heartworm. It is perfectly legal to give a reduced dosage of any drug or pesticide if labeled for use in that crop or animal so don't let anyone tell you it is illegal to do so. You may need to use another product for fleas such as low rate 1/2 full dose Comfortis split on a 3 week schedule .. it will control both adult and juvenile fleas much better than Sentinel. Comfortis contains the active ingredient spinosad which is in the same chemical family but has an alternate mode of action and at 1/2 full dose for your dog's weight it should not be a problem (but keep an eye on things as your dog has a lot of milbemycin in his system). The mode of action differs within the chemical family however.

The caution against giving Atopica ( Cycloporin ) with Sentinel is on the Sentinel label .. maybe your vet should read the product label.  Your comments on why giving cyclosporin with milbemycin is a bad idea is absolutely correct.
