When my dog was a puppy he was doing well in housebreaking, and then I got the flu for about a week, and the people I asked to take care of him didn't let him out of the crate early enough in the morning...so I had a bit too much in my hands when I was back up. What I found worked was, as a seperate thing, teach the dog what a marker means...in other words, teach him that when you ask something, like say "Sit", and he sits, you say "Good boy" and then you treat him/praise him. In fact, I wouldn't treat/praise unless the dog did something good first AND I marked it. After that it was just a matter of letting him out, giving a command to potty, and marking him when he goes. It took a while since he would sometimes just roam around sniffing the grass but the communication was clearer and after a couple of times he learned that when I said "Potty" that was what I wanted of him. Now when I call him and he's not 'done yet' he'll nudge my arm and then go back and do his thing.

Speaking of that, dogs sometimes do it two or three times in succession, so maybe you're not just letting her do it outside long enough. :p

I had a similar problem with a pup I got at 4 mos old. She was kept in a kennel area in my basement when I was not home. There were 2 other dogs, three separate kennel areas fenced with chain link. The other dogs were very clean and only went potty when they were let outside. I never left the dogs in kennels more than 4 hours. However the pup did No 1 and 2 all over the kennel and ran through it, layed in it. She was very dirty. She was kenneled before I got her. So I crated her then when I was out and she was better although still not clean. I persisted a long, long time to take her out to the lawn in one place, praise her when she went, scheduled feed and water. It took several months however and a lot of patience, but she got cured and now at 2 years will not soil in the house or in her crate. However, I could never again kennel her. Good luck. I know your patience is running out but your have to be more stubborn than the dog.

At this stage I would suggest you keep her in a decent sized backyard or fenced in area. Everytime she has to go it should be outside. Bring her in the house whenever you can keep a sharp eye on her. Don't let her have an accident in the house. If she needs to go rush her outside and praise her, treats etc. The suggestion to totally clean your carpets should be done before she is allowed to run around the house. At night if she needs to stay inside make sure she has already went before you crate her, I would keep fresh bedding in her crate at all times. Bottom line, don't let her run free in the house so she can have her "accidents" and you don't know about it until after the fact. Anyway, this is what I would do but good luck no matter what you try!
